Job Description

Responsibilities:

 

Collaborates with the Management Team and implements the Organizational structure of the Certified Home Health Agency by:

·        Defining roles and lines of responsibility and authority in the creation of performance-based job descriptions that establish clear expectations.

·        Conducting interviews and recommending clinicians for hire.

·        Promoting a positive culture of accountability and competency in meeting established agency goals of providing quality patient care through appropriate utilization of staff.

·        Evaluating staffing needs based on statistical management reports including current caseload, productivity reports, case mix weight, and distribution of disciplines provided.

·        Participating in periodic review of operations at management meetings.

·        Providing direction, guidance, and support to Clinical Managers, WOCN, and Nutritionist, through activities including but not limited to:

o  Attendance at team care management meetings at least monthly.

o  Conduct monthly clinical management meetings for exchange of information.

o  Conduct individual performance evaluations for each direct report position holding each accountable for defined performance standards.

o  Interpret agency policies and procedures, nursing standards of practice, CMS and NYSDOH

o  Hold managers accountable to enforce performance standards by all clinicians whose work:

·        Meets productivity.

·        Demonstrates OASIS (Outcome and Assessment Information Set) competency.

·        Demonstrates clinical competency.

·        Complies with timely documentation requirements.

·        Elicits positive patient satisfaction.

·        Demonstrates effective care planning and utilization management.

 

2.     Collaborates with CFO and Finance Department to

·        Resolve reimbursement issues.

·        Participate in budgetary process and implementation,

 

3.     Collaborates with Director of IT and IT Department to

·        Implement operational software.

·        Provide IT support to field clinicians.

·        Identify data required for program analysis.

·        Standardize reports to be reviewed.

 

4.     Collaborates with the Director of Quality Assurance Performance Improvement (QAPI) and Education in the integration of the provision of clinical services to patients though

·        performance improvement activities and education of staff including OASIS walks.

·        Staff participation in care management.

·        Mentorship by clinical staff as an adjunct to orientation of inexperienced staff.

·        Clinical staff performance relative to measured clinical outcomes.

·        Attendance at regularly scheduled QAPI meetings to review agency outcomes as publicly reported and monitored internally.

·        Participation in VNSHS annual agency evaluation.

 

5.     Collaborates with Director of Intake and Service Development to

·        process referrals.

·        identify needs reflected by referral sources.

·        identify trends of referral flow to project corresponding staffing needs.

 

6.     Collaborates with Human Resources Manager to address and coordinate

·        Personnel related conflict resolution.

·        Onboarding and terminations.

 

7.     Participates in Administrative management meetings to

·        Review operations and coordinate standardization of reports to be reviewed.

·        Collaborate with department head colleagues.

·        Communicate issues of concern related to CHHA operations.

·        Report on and support need for services.

 

8.     Keeps current with industry trends through

·        Participation in web seminars presented by regulatory bodies, membership organizations, or related organizations.

·        Communication with like agencies through shared participation in relevant committees.

 

9.     Supports the Agency’s mission and adheres to all applicable federal, state, local laws, and regulations.

 

10. Other duties as assigned.

 

Qualifications:

1.     Registered Professional Nurse currently licensed in New York State.

2.     Bachelor's degree in nursing or other health related field from an accredited college or university; master's degree preferred.

3.     Minimum of two years’ experience in a supervisory capacity in a Certified Home Health Agency preferred OR 2 years of nursing supervisory experience in combination with a master’s degree in nursing, public health, business administration or another health-related field.

4.     Leadership and decision-making ability.

5.     Familiarity with home care and its implementation.

6.     Skilled communicator and autonomous professional, flexible, and innovative.
