Job Description
Responsibilities/Job Duties:
- Provide patient care as outlined in written policies approved by nursing and medical administration
- Provide safe and appropriate medical care to all patients and visitors
- Act as a liaison between nursing staff and medical directors
- Perform comprehensive assessments of patients based on medical history
- Identify, diagnose, and document chronic illnesses and/or diseases in patients
- Provide medical follow-ups, tracking, and reporting of patient health
- Refer patients to clinics or community resources when necessary
- Prescribe medication safely and efficiently
- Write discharge prescription orders on the day of discharge
- Determine the effectiveness of the plan of care
- Manage emergencies by determining the level of risk and initiating emergency care
- Consult with physician as appropriate
- Participate in performance improvement and research activities
- Various administration tasks as needed
Qualifications:
- Master’s degree in Nursing/Nurse Practitioner
- Minimum one year of nursing experience required
- Strong computer skills required, including Word, Excel, and Powerpoint
- Familiar with EMR software
- Nurse Practioner License required
- RN license required
- Ability to stand for long periods of time
- Excellent patient service skills
