Job Description

Responsibilities/Job Duties: 

  • Maintain firm adherence to the resident’s “Bill of Rights”, as they relate to the facility. 
  • To maintain strict adherence to the principles of confidentiality relating to any aspect of the staff–resident relationship. The inappropriate or spontaneous sharing of confidential information with anyone with no legitimate right to such is a serious breach of professional conduct. 
  • To maintain communication and updates from the psychologist regarding any residents receiving psychological services. Also, make referrals to the psychologist if needed. 
  • To offer new ideas or suggestions for change or modification of the role of the Case Manager when such may seem indicated based upon the Case Manager’s ongoing assessment of his/her function in the facility. 
  • Attends workshops and seminars and participates in pertinent in-service programs. 
  • Maintains notes pertinent to the Case Management – resident relationship in addition to any individual contact, which is of meaningful clinical significance, such as the residents’ adjustment to placement. 
  • Assessments and coordination of admission/re-admission from the community, nursing homes, and hospitals. 
  • Coordinate and supervise clinical care for residents 
  • Clear and concise documentation in accordance with facility requirements. 

Qualifications: 

  • Bachelor's Degree, preferably one in behavioral sciences 
  • Prior experience in Long Term Care a PLUS or 2 years experience as a Case Manager 
  • Previous case management and Compucare experience is a plus and will train. 
  • Excellent phone skills and ability to work with families/other care providers in coordinating services for the Residents 
  • Organizational skills, passion, and compassion for the population 
  • Computer literate, visual EMR knowledge 
  • Ability to work cooperatively with multidisciplinary teams
