Job Description

Overview

  • Provide compassionate social work services to patients and families facing serious illnesses.
  • Collaborate with interdisciplinary teams to deliver holistic, patient-centered care.
  • Support emotional, social, and practical needs of patients and caregivers.
  • Facilitate discussions on advance directives and end-of-life planning.
  • Advocate for patient preferences and cultural values in care decisions.
  • Conduct home visits, assessing patient needs and providing tailored support.
  • Maintain compliance with documentation standards and organizational policies.
  • Participate in shared weekend rotations as part of the care team.

Key Responsibilities & Duties

  • Conduct comprehensive assessments to identify patient and caregiver needs.
  • Provide emotional and psychosocial support to patients and families.
  • Assist with navigating healthcare systems and accessing community resources.
  • Collaborate with interdisciplinary teams to create personalized care plans.
  • Document patient interactions and care plans in electronic medical records.
  • Facilitate discussions on care goals and advance planning directives.
  • Provide crisis intervention during acute patient or caregiver challenges.
  • Participate in team meetings and contribute to holistic care strategies.

Job Requirements

  • Master's degree in Social Work (MSW) from an accredited program.
  • NYS LMSW license required; NYS LCSW preferred.
  • Experience in palliative care, hospice, or medical social work preferred.
  • Knowledge of serious illness, grief, and community resources.
  • Strong communication and clinical documentation skills.
  • Ability to work independently and collaboratively within a team.
  • Valid driver’s license and reliable transportation for home visits.
  • Commitment to patient-centered care and cultural sensitivity.
