Overview
- Provide comprehensive nursing care and medical procedures within LPN scope to support client health and well-being.
- Collaborate with healthcare providers, schools, and community organizations to address client medical needs effectively.
- Administer prescribed treatments and medications, ensuring adherence to policies and accurate documentation.
- Respond promptly to medical emergencies, performing necessary triage and follow-up care.
- Assist in integrating physical and behavioral health care within the program framework.
- Maintain accurate and thorough patient records to comply with internal and external regulations.
- Participate in quality improvement initiatives to enhance client outcomes and care processes.
- Support a patient-centered care environment emphasizing confidentiality, comfort, and mutual respect.
Key Responsibilities & Duties
- Administer prescribed medical treatments and medications, ensuring accurate documentation and adherence to protocols.
- Schedule and coordinate client appointments, obtaining necessary medical documentation beforehand.
- Act as liaison with external healthcare providers and community organizations regarding client medical needs.
- Perform general nursing care, including vital signs recording, phlebotomy, and specimen preparation.
- Respond to medical emergencies, providing triage and necessary interventions within LPN scope.
- Maintain inventory and requisition supplies to ensure readiness for client care.
- Participate in agency quality improvement projects to optimize client outcomes.
- Complete all tasks within the scope of NYS LPN license as directed by RN or Nursing Leadership.
Job Requirements
- Active LPN nursing license in New York State and current CPR certification.
- Bachelor of Science degree preferred, with strong communication and organizational skills.
- Proficiency in data management and collaboration, ensuring effective care delivery.
- Ability to respond swiftly to emergencies, performing necessary interventions and protocols.
- Physical capability to perform nursing duties, including lifting and repositioning clients safely.
