Overview

  • Provide professional rehabilitation therapy to patients in their homes, ensuring individualized, goal-oriented therapy programs are implemented effectively.
  • Collaborate with healthcare teams to ensure coordinated care and optimal resource utilization for patients.
  • Perform patient evaluations within specified timeframes, focusing on holistic assessments and measurable goals.
  • Document patient progress, reassessments, and discharge summaries precisely and timely.
  • Educate patients and caregivers on home programs tailored to individual needs.
  • Supervise Physical Therapy Assistants, ensuring interventions are performed effectively and co-signatures are timely.
  • Adhere to organizational standards and competencies, demonstrating flexibility with job responsibilities.
  • Participate in discharge planning, addressing problem areas and recommending further services as needed.

Key Responsibilities & Duties

  • Conduct thorough patient evaluations, developing individualized treatment plans with measurable short and long-term goals.
  • Prepare and maintain accurate documentation, including progress notes and reassessments, supporting the need for skilled care.
  • Coordinate with physicians and healthcare teams to ensure effective care delivery and resource utilization.
  • Provide comprehensive education to patients and caregivers, ensuring understanding of treatment plans and home programs.
  • Supervise and assess Physical Therapy Assistants, ensuring interventions align with treatment plans.
  • Respond promptly to communications and update schedules accurately through system applications.
  • Participate in discharge planning, documenting patient status, outcomes, and recommendations for further interventions.
  • Adhere to organizational standards, demonstrating flexibility and commitment to patient care excellence.

Job Requirements

  • Doctoral Degree in Physical Therapy (PT) or equivalent educational background.
  • Minimum of six months of professional experience in physical therapy; three years preferred.
  • Active Physical Therapist License and AHA Basic Health Care Life Support HCP Certification.
  • Proficiency in PC usage and relevant software applications.
  • Strong documentation skills, ensuring timely and precise submission of required records.
  • Ability to lift, push, and pull a minimum of 50 lbs., standing for extended periods.
  • Effective communication skills for patient education and team coordination.
