Job Description

Overview

  • Serve as a Remote UAS Enrollment Nurse, facilitating member enrollment and service planning.
  • Collaborate with members to assess needs and develop personalized care plans.
  • Conduct field visits as required, approximately 15% of the time.
  • Work primarily remotely with flexible scheduling for in-person meetings.
  • Utilize knowledge of UAS to evaluate member requirements effectively.
  • Ensure compliance with state regulations and organizational standards.
  • Participate in hybrid training sessions to enhance professional skills.
  • Contribute to a supportive and collaborative team environment.

Key Responsibilities & Duties

  • Meet with prospective members to discuss enrollment options and services.
  • Review NYIA assessments and develop tasking tools for service allocation.
  • Create and present Person-Centered Service Plans (PCSP) to members.
  • Conduct field visits to address specific member needs and requests.
  • Collaborate with interdisciplinary teams to ensure comprehensive care.
  • Maintain accurate documentation of member interactions and plans.
  • Provide exceptional customer service and support to members.
  • Stay updated on industry best practices and regulatory changes.

Job Requirements

  • Bachelor of Science in Nursing (BSN) degree required.
  • Active Registered Nurse (RN) license in the State of New York.
  • Minimum of 1 year of relevant nursing experience.
  • Proficiency in UAS and related healthcare assessment tools.
  • Strong interpersonal and communication skills.
  • Ability to work remotely and adapt to flexible scheduling.
  • Willingness to conduct field visits as needed.
