Overview
- Lead clinical operations within a residential treatment facility serving youth aged 12-18 with complex mental health diagnoses.
- Collaborate with interdisciplinary teams to ensure effective therapeutic and educational support for residents.
- Develop and enhance transitional and permanency programs to improve outcomes and reduce re-referrals.
- Provide clinical supervision to teams focused on family dynamics, educational success, and socialization needs.
- Oversee discharge planning and ensure successful transitions into community settings.
- Maintain compliance with all relevant regulations and guidelines, ensuring best practices in care delivery.
- Participate in clinical meetings and provide updates to administration on program progress.
- Establish collaborative relationships with local resources and systems impacting youth and families.
Key Responsibilities & Duties
- Supervise Transitional Coordinators and Family Connections Specialists, ensuring compliance with documentation and care standards.
- Lead joint meetings with community-based programs to coordinate support services for families.
- Oversee the utilization of transitional funds in compliance with policies and guidelines.
- Provide direct transitional coordinator services as needed, ensuring smooth operations during vacancies.
- Support teams in navigating systems such as OCFS, DSS/ACS, DOE, and Medicaid.
- Ensure educational needs are met through regular interaction with the in-house school system.
- Participate in treatment meetings and contribute to the development of care plans for residents.
- Manage the overall direction and evaluation of transitional and family support entities within the program.
Job Requirements
- Master's degree in Social Work or related field with LCSW certification.
- Minimum of 2 years of experience providing direct services to children with SED.
- Supervisory experience in the social service sector, preferably with residential programs.
- Strong communication skills and ability to write effectively.
- Willingness to travel to families’ homes and work one weekend day per week.
- Experience with EMR systems and familiarity with therapeutic crisis intervention techniques.
- Ability to manage high-acuity populations and align with the mission of the organization.
- Knowledge of local resources and systems impacting youth and families.
