Job Description

Overview

  • Provide comprehensive primary care services to patients, focusing on holistic health outcomes and value-based care.
  • Work in a mission-driven environment serving underserved communities with dedicated support from social workers and care teams.
  • Engage in patient-centered care with manageable patient panels and structured appointment times.
  • Collaborate with hospitalists and specialists to ensure continuity and quality of care for patients.
  • Participate in quality improvement initiatives and risk management programs to enhance patient outcomes.
  • Actively contribute to the positive culture and leadership within the medical center.
  • Utilize technology effectively for documentation and communication within the healthcare team.
  • Adhere to departmental goals, regulatory compliance, and quality care standards.

Key Responsibilities & Duties

  • Provide care for patients with acute and chronic illnesses, ensuring comprehensive treatment and management.
  • Lead care teams, including medical assistants and coordinators, to deliver patient-centered care.
  • Engage with transitional care teams for patients unable to visit the office, ensuring continuity of care.
  • Supervise and collaborate with Advanced Practice Practitioners, maintaining accountability for their actions.
  • Participate in clinical meetings, quality management programs, and risk assessments to improve healthcare delivery.
  • Promote patient health through education, health promotion, and care coordination.
  • Recruit patients and providers to support the growth and success of the medical center.
  • Ensure compliance with medical guidelines, policies, and procedures while maintaining high-quality patient care.

Job Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) in Internal Medicine, Family Medicine, or Geriatrics.
  • Active State Medical License or ability to obtain one within the required timeframe.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics, with maintenance of certification.
  • Current DEA number for Schedule II-V controlled substances.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross.
  • Proficient in Microsoft Office Suite and other healthcare-related software tools.
  • Strong commitment to patient-centered care and mission-driven healthcare delivery.
  • Ability to travel locally and nationally for training and recruitment purposes as needed.
