Nurse Practitioner – Geriatric Primary Care

Key Responsibilities & Duties

• Deliver comprehensive geriatric primary care in home and virtual settings.
• Perform diagnostic and therapeutic procedures within scope of practice.
• Collaborate with clinical teams to ensure seamless patient care delivery.
• Utilize electronic medical records for documentation and care coordination.
• Provide consultations and support for allied health professionals.
• Participate in quality improvement initiatives to enhance care standards.
• Maintain compliance with regulatory and licensing requirements.

Job Requirements

• Master of Science in Nursing from an accredited institution.
• Active unrestricted Nurse Practitioner license in New York State.
• Minimum of 1 year of practice; 3 years preferred.
• DEA certification required for prescribing medications.
• Proficiency in electronic medical records and telemedicine platforms.
• Strong communication skills for patient and team interactions.
• Willingness to obtain licensure in multiple states as needed.
