Overview
- Provide direct mental health services and depression care management in a hybrid work environment.
- Collaborate with interdisciplinary teams to deliver comprehensive patient care and support.
- Utilize standardized tools for patient assessment and follow agency guidelines for behavioral health diagnoses.
- Participate in clinical supervision and contribute to continuous quality improvement initiatives.
- Document patient care in electronic health records promptly and accurately.
- Offer referrals for long-term counseling and other essential services as needed.
- Engage in professional development and maintain required certifications and licensure.
- Work with diverse populations, including HIV patients, homeless individuals, and children.
Key Responsibilities & Duties
- Deliver collaborative care services, including telephonic outreach and management of assigned cases.
- Coordinate care for patients in primary care settings and provide direct clinical services.
- Utilize PHQ-9 and other tools for patient assessment and care planning.
- Ensure timely completion of patient documentation and electronic encounters.
- Participate in department and organizational CQI projects to enhance care quality.
- Design and implement clinical pathways and protocols for chronic condition treatment.
- Develop linkages with community organizations and resources for patient support.
- Provide psychiatric consultations and collaborate with healthcare teams.
Job Requirements
- Master’s Degree in Social Work or related field required.
- NYS licensure as LMSW, LCSW, PhD, or PsyD preferred.
- Experience working with diverse populations, including HIV, homeless, and mentally ill individuals.
- Proficiency in electronic health records and Windows-based systems.
- Bilingual Spanish skills preferred, with additional compensation offered.
- Current BCLS and Infection Control certifications required.
- Strong knowledge of behavioral medicine and evidence-based treatments for depression.
- Ability to make independent judgments and manage caseload effectively.
