Social Worker – Hybrid

Overview

• Provide direct depression care management and clinical services to patients in a hybrid work environment.
• Collaborate with healthcare teams to deliver high-quality mental health services and support.
• Utilize standardized tools for patient assessments and care planning.
• Engage in telephonic outreach and management of assigned cases effectively.
• Participate in clinical supervision and continuous quality improvement projects.
• Work with diverse populations, including HIV, homeless, mentally ill, elderly, children, and adolescents.
• Maintain patient confidentiality and adhere to organizational policies and standards.
• Demonstrate professional and respectful communication with patients, families, and staff.

Key Responsibilities & Duties

• Provide collaborative care services, including mental health support and coordination of care in primary settings.
• Document patient care activities promptly and ensure timely closure of electronic encounters.
• Offer referrals for long-term counseling and additional services like housing assistance.
• Utilize evidence-based tools for patient assessments and behavioral health workflows.
• Manage caseloads effectively using reporting tools for self-management.
• Participate in organizational and departmental quality improvement initiatives.
• Engage in clinical supervision and interdisciplinary team collaboration.
• Design and implement clinical pathways for chronic condition treatment protocols.

Job Requirements

• Master’s Degree in Social Work or related field required.
• NYS licensure as LMSW, LCSW, PhD, or PsyD preferred.
• Experience working with diverse populations and healthcare settings preferred.
• Proficiency in electronic health records (EHR) and MS Office applications.
• Bilingual skills in Spanish preferred, with additional compensation available.
• Knowledge of behavioral medicine and evidence-based treatments for depression.
• Strong communication, organizational, and interpersonal skills required.
• Ability to work in a hybrid environment with flexible scheduling options.
