Overview

• Provide comprehensive care management services to members, ensuring effective communication and coordination of benefits.
• Act as the primary liaison between members and the plan, addressing their needs and concerns.
• Utilize bilingual Bengali skills to enhance member engagement and understanding.
• Work in a hybrid environment with a mix of onsite and remote days.
• Participate in orientation and training sessions to ensure role readiness.
• Conduct member assessments and develop person-centered service plans.
• Collaborate with multidisciplinary teams to optimize member care.
• Maintain accurate and timely documentation in member charts.

Key Responsibilities & Duties

• Communicate monthly with members to follow up on their care plans.
• Coordinate member care with families and vendor agencies effectively.
• Document all interactions and updates in member charts promptly.
• Participate in weekly team meetings to discuss member progress.
• Conduct assessments and clinical reviews as required.
• Ensure compliance with organizational policies and procedures.
• Provide bilingual support to enhance communication with Bengali-speaking members.
• Perform basic Excel tasks during documentation and reporting.

Job Requirements

• Bachelor's degree in a relevant field or equivalent experience.
• Minimum of 1 year of experience in Managed Long-Term Care (MLTC).
• Proficiency in Bengali language for effective member communication.
• Basic Excel skills including document management and data sorting.
• Strong knowledge of member assessment and clinical review procedures.
• Ability to work in a hybrid environment with onsite and remote days.
• Excellent communication skills with diverse age and socioeconomic groups.
• Familiarity with care management documentation and reporting standards.
