Job Description

Overview

  • Provide high-quality nursing care to residents in a long-term care setting.
  • Perform duties as a Licensed Practical Nurse (LPN) ensuring patient safety and satisfaction.
  • Collaborate with Registered Nurses and Clinical Care Managers for optimal care delivery.
  • Work during the night shift from 11 PM to 7 AM, including rotating weekends.
  • Engage in in-person interviews with a single-step process for hiring.
  • Contribute to a supportive and professional healthcare environment.
  • Utilize your expertise in geriatric care to enhance patient outcomes.
  • Maintain compliance with all healthcare regulations and standards.

Key Responsibilities & Duties

  • Administer medications and treatments as prescribed by physicians.
  • Monitor patient conditions and report changes to the healthcare team.
  • Document patient care activities accurately and timely.
  • Assist patients with daily living activities and mobility needs.
  • Perform charge nurse responsibilities as assigned.
  • Ensure compliance with infection control protocols and procedures.
  • Provide emotional support and education to patients and families.
  • Participate in team meetings and training sessions for professional development.

Job Requirements

  • Valid NYS Licensed Practical Nurse (LPN) certification.
  • Minimum of 2 years of experience in long-term care settings.
  • Graduate of an approved program in practical nursing.
  • Basic Cardiac Life Support (BCLS) and IV certification required.
  • Commitment to working every other weekend.
  • Strong communication and interpersonal skills.
  • Ability to work on-site during specified hours.
  • Proficiency in patient care documentation and healthcare software.
