Job Description

Overview

  • Provide social work services focusing on discharge planning and compliance with Medicare regulations.
  • Work on-site Friday through Monday, with 10-hour shifts.
  • Utilize Epic systems for documentation and workflow management.
  • Ensure compliance with Medicare compliance letters, including MOONs and IMMs.
  • Collaborate with healthcare teams to support patient transitions and care plans.
  • Engage with patients and families to address social and emotional needs.
  • Maintain accurate and timely documentation of all interventions and plans.

Key Responsibilities & Duties

  • Develop and implement discharge plans tailored to individual patient needs.
  • Coordinate with multidisciplinary teams to ensure seamless transitions of care.
  • Prepare and manage Medicare compliance letters, including MOONs and IMMs.
  • Utilize Epic systems for patient documentation and workflow processes.
  • Provide counseling and support to patients and families during care transitions.
  • Ensure adherence to regulatory and organizational standards in all activities.
  • Monitor and evaluate patient progress and adjust plans as necessary.
  • Participate in training and development to maintain professional competencies.

Job Requirements

  • Master of Social Work (MSW) degree required; LMSW or LCSW certification preferred.
  • Minimum of 2 years of social work experience in healthcare settings.
  • Proficiency with Epic systems and Medicare compliance processes.
  • Strong knowledge of discharge planning and care management practices.
  • Excellent communication and interpersonal skills for patient and team interactions.
  • Ability to work on-site with flexible scheduling, including 10-hour shifts.
