Key Responsibilities & Duties

  • Diagnose, treat, and manage acute and chronic health conditions in adult patients.
  • Lead care teams, including medical assistants and care coordinators, to deliver optimal patient care.
  • Develop and implement personalized health plans to achieve patient health goals.
  • Conduct patient consultations, ensuring thorough assessment and documentation.
  • Collaborate with specialists and hospitalists for comprehensive patient management.
  • Participate in quality improvement initiatives and risk management programs.
  • Engage in community outreach and patient recruitment activities.
  • Maintain compliance with medical licensing and certification requirements.

Job Requirements

  • Doctor of Medicine (MD) degree with board certification in Internal Medicine, Family Medicine, or Geriatrics.
  • Active state medical license and DEA certification for controlled substances.
  • Minimum of 1 year of experience, with 5 years preferred in primary care settings.
  • Proficiency in Microsoft Office Suite and healthcare documentation systems.
  • Strong commitment to patient-centered care and value-based healthcare practices.
  • Ability to work onsite Monday through Friday during standard business hours.
  • Basic Life Support (BLS) certification within the first 90 days of employment.
  • Excellent communication skills and fluency in English, with additional language proficiency as needed.
