Overview

- Collaborate with interdisciplinary teams, including primary care and behavioral health professionals, to deliver integrated patient care.
- Educate patients and their families about mental health conditions, treatment options, and medication adherence.
- Ensure accurate and timely documentation of patient encounters and treatment plans in the Electronic Health Record (EHR) system.
- Participate in quality improvement initiatives to enhance patient outcomes and service delivery.

Key Responsibilities & Duties

Job Requirements
- Master of Science in Nursing with specialization in Psychiatric Mental Health.
- Licensed Psychiatric Nurse Practitioner in New York State with active certification.
- Minimum of 2 years of experience in outpatient behavioral health settings; 4 years preferred.
- Proficiency in conducting psychiatric evaluations, medication management, and treatment planning.
- Experience collaborating with interdisciplinary teams to deliver integrated patient care.
- Familiarity with Electronic Health Record (EHR) systems for accurate documentation.
