Overview

  • Provide outpatient psychiatric services in a Federally Qualified Health Center (FQHC) setting, ensuring comprehensive mental health care for diverse patient populations.
  • Conduct psychiatric evaluations, medication management, and treatment planning to address various mental health conditions effectively.
  • Collaborate with interdisciplinary teams, including primary care and behavioral health professionals, to deliver integrated patient care.
  • Educate patients and their families about mental health conditions, treatment options, and medication adherence.
  • Ensure accurate and timely documentation of patient encounters and treatment plans in the Electronic Health Record (EHR) system.
  • Participate in quality improvement initiatives to enhance patient outcomes and service delivery.
  • Stay updated with the latest psychiatric practices and guidelines to provide evidence-based care.
  • Contribute to a supportive and collaborative work environment within the healthcare team.

Key Responsibilities & Duties

  • Conduct comprehensive psychiatric evaluations to assess and diagnose mental health conditions.
  • Develop individualized treatment plans, including medication management and therapeutic interventions.

Job Requirements

  • Master of Science in Nursing with specialization in Psychiatric Mental Health.
  • Licensed Psychiatric Nurse Practitioner in New York State with active certification.
  • Minimum of 2 years of experience in outpatient behavioral health settings; 4 years preferred.
  • Proficiency in conducting psychiatric evaluations, medication management, and treatment planning.
  • Experience collaborating with interdisciplinary teams to deliver integrated patient care.
  • Familiarity with Electronic Health Record (EHR) systems for accurate documentation.
  • Strong communication skills for patient education and team collaboration.
