Overview
- Coordinate and evaluate home care needs for patients, ensuring effective care plans and positive clinical outcomes.
- Conduct initial assessments and create interdisciplinary patient-centered home health plans of care.
- Collaborate with physicians and caregivers to update and implement care plans.
- Provide direct bedside nursing care and manage acute needs for discharged patients.
- Ensure compliance with insurance requirements and coordinate activities for individual care plans.
- Work full-time, Monday to Friday, with occasional weekend on-call duties.
- Field-based role requiring visits to patients' homes or facilities within the Brooklyn area.
- Engage in virtual and in-person interviews during the hiring process.
Key Responsibilities & Duties
- Perform initial patient assessments and develop comprehensive home health care plans.
- Coordinate and manage care services to minimize risks of rehospitalization.
- Provide bedside nursing care to patients with acute needs at home.
- Collaborate with physicians, caregivers, and family members to ensure effective care delivery.
- Review patient insurance information and ensure compliance with coverage requirements.
- Participate in interdisciplinary team discussions to optimize patient care plans.
- Manage on-call duties one weekend per month, providing support as needed.
- Complete pharmacy tests and training requirements as part of onboarding.
Job Requirements
- Associate of Applied Science (AAS) degree required; Bachelor of Science in Nursing (BSN) strongly preferred.
- Registered Nurse (RN) certification mandatory.
- Minimum of 2 years of experience in hospital, acute care, Med/Surg, COC, CHHA, or nursing home settings.
- Field-based role requiring a valid driver’s license and reliable transportation.
- Proficiency in case management and interdisciplinary care coordination.
- Ability to conduct virtual and in-person interviews during the hiring process.
- Strong communication skills to interact with patients, families, and healthcare professionals.
