Job Description

Overview

  • Provide high-quality healthcare services as a Nurse Practitioner in a dynamic clinical setting.
  • Conduct comprehensive health assessments, including physical exams and medical histories.
  • Develop individualized care plans in collaboration with interdisciplinary teams.
  • Manage chronic conditions and acute illnesses with timely interventions.
  • Order and interpret diagnostic tests to aid in diagnosis and treatment planning.
  • Prescribe medications and treatments within scope and regulations.
  • Educate residents and families on health maintenance and disease prevention strategies.
  • Participate in quality improvement initiatives to enhance care outcomes.
  • Maintain accurate documentation in electronic medical records.

Key Responsibilities & Duties

  • Conduct physical examinations and medication reconciliation for residents.
  • Collaborate with interdisciplinary teams to create personalized care plans.
  • Manage chronic and acute health conditions with appropriate interventions.
  • Order and interpret diagnostic tests for accurate treatment planning.
  • Prescribe medications and treatments following state regulations.
  • Educate residents and families on health strategies and self-care practices.
  • Participate in chronic care management service initiation.
  • Contribute to quality improvement initiatives and interdisciplinary meetings.
  • Ensure accurate documentation in electronic health records.

Job Requirements

  • Master’s degree in Nursing and completion of an accredited Nurse Practitioner program.
  • Unrestricted New York State Nurse Practitioner license.
  • DEA permit to prescribe medications within practice scope.
  • Eligible to provide reimbursable services under Medicare and Medicaid.
  • Minimum of 2 years of clinical experience, preferably in long-term care.
  • Proficiency with eClinicalWorks (eCW) electronic health records.
  • Strong knowledge of chronic care management practices.
  • Excellent communication and collaboration skills.
  • Ability to travel to multiple sites within a 30-mile radius.
