Overview
- Serve as a Family Nurse Practitioner specializing in Women's Health and Primary Care.
- Provide comprehensive care to underserved populations with complex medical needs.
- Conduct well-woman exams, preventive screenings, and chronic disease management.
- Collaborate with interdisciplinary teams to ensure high-quality patient outcomes.
- Engage in both clinic and in-home patient visits for holistic care delivery.
- Utilize evidence-based guidelines to address primary care and gynecological concerns.
- Support patients through pregnancy, menopause, and chronic condition management.
- Maintain accurate documentation within the Electronic Medical Record (EMR) system.
Key Responsibilities & Duties
- Perform comprehensive history and physical exams for risk adjustment and assessments.
- Provide full-spectrum contraceptive counseling and services, including IUD insertions.
- Diagnose and treat sexually transmitted infections and provide sexual health counseling.
- Develop individualized care plans for chronic conditions like hypertension and diabetes.
- Collaborate with specialists for seamless prenatal and specialty care coordination.
- Screen for social determinants of health, including domestic violence and substance use.
- Engage in Tele-Health communications for accessible patient care delivery.
- Ensure compliance with New York State laws and clinical practice guidelines.
Job Requirements
- Master of Science in Nursing with FNP certification.
- Current RN licensure and DEA registration in New York State.
- 3–5 years of experience in Women's Health and Primary Care.
- Proficiency in phlebotomy, immunizations, and office-based procedures.
- Experience working with underserved populations facing healthcare barriers.
- Fluency in an additional language is highly preferred.
- Strong clinical judgment and ability to collaborate effectively with teams.
