Key Responsibilities & Duties

• Diagnose, treat, and manage acute and chronic conditions in adult patients, prioritizing quality and patient-centered care.
• Lead care teams to deliver exceptional medical services and ensure adherence to clinical guidelines and standards.
• Participate in risk management and quality improvement programs to enhance patient outcomes and satisfaction.
• Coordinate with hospitalists to ensure continuity of care during hospital admissions and transitions.

• Document patient interactions and care plans accurately and efficiently using electronic medical records systems.

Job Requirements

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree is required.
• Board certification in Internal Medicine, Family Medicine, or Geriatrics is mandatory.
• Active state medical license or eligibility to obtain one within the hiring timeframe.
• Proficiency in electronic medical records systems for efficient patient care management.
• Experience with value-based care models and accountability for patient outcomes is preferred.
• Strong leadership and communication skills to effectively guide care teams and foster collaboration.
• Basic Life Support (BLS) certification required within 90 days of employment.
