Overview
- Provide nursing care and health education in an adult medical day care setting.
- Collaborate with a multidisciplinary team to support registrants' health and wellness.
- Perform nursing assessments and develop individualized treatment plans for registrants.
- Monitor medications and ensure compliance with established inventory standards.
- Provide health counseling and lead group health education sessions.
- Act as a liaison between registrants, their physicians, and the program's Medical Director.
- Participate in quality assurance and performance improvement activities.
- Maintain confidentiality and adhere to security policies regarding health information.
- Support registrants with personal care and mobility assistance as needed.
Key Responsibilities & Duties
- Conduct nursing assessments and develop, implement, and evaluate treatment plans.
- Monitor registrants' medications and ensure proper inventory management.
- Handle medical emergencies and educate staff on safety procedures.
- Maintain a clean and safe environment, including medical equipment upkeep.
- Provide individual and group health counseling and education sessions.
- Supervise nursing students and volunteers, ensuring proper training and guidance.
- Collaborate with the interdisciplinary team to enhance registrants' care.
- Participate in staff meetings and contribute to program development.
- Assist registrants with mobility, meals, and transportation as needed.
Job Requirements
- Associate of Applied Science (AAS) degree from an accredited nursing school.
- Current NYS Registered Nurse license and UAS-NY certification preferred.
- 3-5 years of recent experience in Med/Surg, Long Term Care, or Community Health.
- Proficiency in MS Word and excellent documentation skills.
- Strong communication skills and ability to work with elderly populations.
- Physical ability to assist registrants with mobility and equipment handling.
- Commitment to maintaining confidentiality and adhering to security policies.
- Warm personality and dedication to fostering registrants' well-being.
- Experience in interdisciplinary team collaboration and program development.
