Job Description

Overview

  • Provide direct support to individuals, ensuring their well-being and safety during daily activities and medical appointments.
  • Advocate for individuals' healthcare needs, ensuring they receive appropriate care and follow-up during medical visits.
  • Support individuals with personal care, recreational programs, and household tasks to enhance their quality of life.
  • Maintain a clean, safe, and organized living environment, adhering to health and safety regulations.
  • Participate in interdisciplinary treatment meetings to develop and implement individualized treatment plans.
  • Complete required training programs, including SCIP and Medication Administration, to ensure professional competency.
  • Work various shifts, including evenings and overnights, to meet the needs of residents effectively.
  • Develop positive relationships with residents, families, staff, and healthcare providers to foster collaboration.

Key Responsibilities & Duties

  • Coordinate and attend medical appointments, ensuring safe transportation and advocacy for residents.
  • Document medical appointments, treatments, and medication administration accurately for effective communication.
  • Assist residents with daily activities, including personal care, recreational programs, and household tasks.
  • Ensure adherence to health and safety regulations, maintaining a safe living environment for residents.
  • Participate in interdisciplinary meetings to contribute to individualized treatment plans for residents.
  • Complete required certifications and training programs, applying learned skills in daily responsibilities.
  • Provide crisis intervention and support as needed, ensuring residents' safety and well-being.
  • Develop and maintain professional relationships with residents, families, and healthcare providers.

Job Requirements

  • High School diploma or GED equivalent required for eligibility.
  • Valid Driver's License to drive in New York State is mandatory.
  • One year of experience working with OPWDD populations is preferred.
  • Ability to lift 50 pounds and assist individuals with movement.
  • Completion of OPWDD Medication Certification Course, CPR, First Aid, and SCIP-R training required.
  • Flexibility to work various shifts, including evenings and overnights.
  • Strong documentation and communication skills for effective reporting.
