Job Description

Overview

  • Provide psychosocial assessments to evaluate patients' mental health, social, and environmental needs.
  • Develop and implement individualized care plans in collaboration with interdisciplinary teams.
  • Offer crisis intervention, emotional support, and counseling to patients and families.
  • Coordinate discharge planning, including placement, home services, and follow-up care.
  • Connect patients with community resources such as housing, financial assistance, and behavioral health services.
  • Advocate for patients to ensure access to appropriate care and services.
  • Collaborate with physicians, nurses, and case managers to support continuity of care.
  • Assist with care transitions to promote safe and cost-effective outcomes.
  • Document assessments, interventions, and care plans in accordance with hospital policies and regulations.

Key Responsibilities & Duties

  • Conduct psychosocial assessments to identify patients' needs and develop care strategies.
  • Provide emotional support and counseling to patients and their families.

Job Requirements

  • Master’s Degree in Social Work (MSW) is required.
  • Licensed Clinical Social Worker (LCSW) certification is preferred.
  • Basic Life Support (BLS) or CPR certification may be required.
  • Minimum of 2 years of experience in social work or related field.
  • Strong knowledge of community resources and care coordination practices.
  • Proficiency in documentation and compliance with healthcare regulations.
  • Ability to work collaboratively within interdisciplinary teams.
  • Excellent communication and advocacy skills to support patient needs.
