Overview
- Coordinate and manage patient care from admission through discharge in a hospital setting.
- Conduct comprehensive assessments to identify clinical, psychosocial, and discharge planning needs.
- Develop and implement individualized care plans in collaboration with interdisciplinary teams.
- Lead discharge planning efforts, ensuring safe and cost-effective transitions of care.
- Collaborate with nursing, physicians, social work, and ancillary departments to optimize patient outcomes.
- Monitor patient progress and adjust care plans based on clinical status and resource needs.
- Facilitate utilization reviews to ensure compliance with payer guidelines and appropriate care levels.
- Coordinate post-acute services including home health, skilled nursing, and rehab resources.
Key Responsibilities & Duties
- Educate patients and families on care plans, treatment options, and post-discharge instructions.
- Advocate for patients to ensure access to appropriate services and continuity of care.
- Maintain accurate and timely documentation in accordance with hospital policies and standards.
- Participate in interdisciplinary rounds and contribute to care coordination strategies.
- Ensure compliance with CMS, Joint Commission, and hospital-specific case management standards.
- Collaborate with healthcare teams to optimize patient care and discharge outcomes.
- Monitor resource utilization to ensure cost-effective care delivery.
- Provide leadership in discharge planning and post-acute care coordination.
Job Requirements
- Active RN license in Colorado or Compact State required.
- Associate of Science (AS) in Nursing; BSN preferred but not mandatory.
- Case Management certification (e.g., CCM) preferred.
- Minimum 2 years of acute care or case management experience required.
- Strong knowledge of utilization review, discharge planning, and payer requirements.
- Ability to work on-site with a set schedule of Thursday to Monday.
- Proficiency in care coordination and interdisciplinary collaboration.
- Excellent communication and documentation skills in a healthcare setting.
