Overview

- Perform Licensed Practical Nurse (LPN) duties under the supervision of a Registered Nurse or Clinical Care Manager.

Key Responsibilities & Duties
- Administer medications and treatments as prescribed, ensuring accuracy and patient safety.
- Monitor patient health, recording vital signs and reporting changes to the supervising nurse.
- Assist with daily living activities and provide emotional support to patients.
- Maintain accurate patient records and documentation in compliance with healthcare standards.
- Collaborate with healthcare team members to develop and implement patient care plans.
- Perform IV therapy and other specialized nursing procedures as required.
- Act as a charge nurse when assigned, overseeing patient care and team coordination.
- Participate in training sessions and stay updated on best practices in geriatric care.
Job Requirements
- Valid Licensed Practical Nurse (LPN) license in the state of New York.
- Associate of Science (AS) degree in Practical Nursing from an accredited program.
- Minimum of 2 years of experience in geriatric patient care.
- Basic Cardiac Life Support (BCLS) certification and IV therapy certification required.
- Strong commitment to patient care and ability to work on-site.
- Willingness to work every other weekend to ensure consistent care delivery.
- Excellent communication and teamwork skills to collaborate effectively with healthcare professionals.
