Job Description

Overview

  • Provide clinical expertise to Medicare members, pharmacies, and healthcare providers to enhance medication adherence and address Statin Use in Persons with Diabetes (SUPD) gaps.
  • Conduct Comprehensive Medication Reviews (CMRs) to improve members' understanding of their medications and overall health.
  • Collaborate with pharmacies and providers to resolve adherence challenges and ensure optimal medication use.
  • Offer counseling and guidance to Medicare members regarding medication management and pharmacy benefits.
  • Utilize clinical knowledge to identify and address potential drug interactions, adverse reactions, and dose adjustments.
  • Support Pharmacy Quality Specialists with clinical insights to improve service delivery and outcomes.
  • Engage members through outreach efforts to remove barriers to medication adherence and enhance health outcomes.
  • Ensure compliance with state pharmacy regulations and standards while delivering high-quality care.

Key Responsibilities & Duties

  • Perform outreach to Medicare members to address medication adherence and SUPD gaps effectively.
  • Conduct CMRs to provide members with a comprehensive understanding of their medications and health status.
  • Collaborate with healthcare providers and pharmacies to resolve adherence issues and optimize medication regimens.
  • Send provider interventions to address drug interactions, adverse reactions, and adherence concerns.
  • Provide counseling to Medicare members regarding their medications and pharmacy benefits.
  • Offer clinical support to Pharmacy Quality Specialists to enhance service quality and member satisfaction.
  • Ensure compliance with pharmacy regulations and maintain high standards of clinical practice.
  • Utilize clinical expertise to identify and address medication-related issues proactively.

Job Requirements

  • Doctor of Pharmacy (PharmD) degree from an accredited institution.
  • Active NC state pharmacy license and residency within the state.
  • Minimum of 3 years of experience in a clinical pharmacy role; 5 years preferred.
  • Proficiency in conducting Comprehensive Medication Reviews and providing clinical counseling.
  • Strong knowledge of Medicare pharmacy benefits and adherence strategies.
  • Ability to collaborate effectively with healthcare providers and pharmacies.
  • Excellent communication skills to engage members and address medication-related concerns.
  • Commitment to maintaining compliance with state pharmacy regulations and standards.
