Job Description

Overview

  • Provide clinical nutrition services to residents in long-term and sub-acute care units.
  • Collaborate with interdisciplinary teams to support resident health and wellness goals.
  • Conduct comprehensive nutrition assessments and develop individualized care plans.
  • Ensure compliance with regulatory requirements and documentation standards.
  • Educate residents, patients, and families on nutrition-related topics.
  • Participate in care plan meetings and contribute to interdisciplinary discussions.
  • Perform meal rounds to assess quality, safety, and satisfaction.
  • Work onsite in a healthcare setting with occasional weekend shifts.

Key Responsibilities & Duties

  • Conduct nutrition assessments for residents in long-term and sub-acute care units.
  • Develop, implement, and monitor individualized nutrition care plans.
  • Document medical records accurately and in compliance with MDS 3.0 standards.
  • Provide nutrition education to residents, patients, and family members.
  • Collaborate with nursing, medical, and rehabilitation staff to align care goals.
  • Participate in interdisciplinary care plan meetings to ensure comprehensive care.
  • Conduct meal rounds to ensure quality, safety, and resident satisfaction.
  • Support the Food and Nutrition Department's objectives and initiatives.

Job Requirements

  • Registered Dietitian credential from the American Dietetic Association (ADA).
  • Minimum of two years of clinical experience in long-term care preferred.
  • Knowledge of MDS 3.0 and regulatory documentation requirements.
  • Strong communication and collaboration skills within interdisciplinary teams.
  • Ability to educate residents and families on nutrition-related topics.
  • Proficiency in medical record documentation and compliance standards.
  • High School Diploma or GED required; advanced education preferred.
  • Commitment to providing high-quality nutrition care in an onsite setting.
