Overview
- Provide professional rehabilitation services to patients in their home settings, ensuring individualized care plans and high-quality therapy programs.
- Evaluate patients comprehensively to develop goal-oriented treatment plans with measurable outcomes for improved functional abilities.
- Collaborate with interdisciplinary teams and physicians to coordinate patient care and ensure effective therapy delivery.
- Maintain accurate and compliant documentation, including progress notes, reassessments, and discharge summaries.
- Monitor patient progress, adjusting treatment plans as necessary to achieve desired therapy goals.
- Supervise Physical Therapist Assistants (PTAs) and ensure performance assessments and co-signatures are completed timely.
- Support discharge planning by providing recommendations for continued care or services post-treatment.
Key Responsibilities & Duties
- Conduct holistic patient evaluations within required timeframes to assess conditions and therapy needs.
- Develop individualized treatment plans with measurable short- and long-term outcomes tailored to patient goals.
- Deliver skilled physical therapy services according to physician orders and established care plans.
- Ensure therapy visits align with prescribed frequency and duration for optimal patient outcomes.
- Collaborate with healthcare professionals to ensure coordinated care and effective communication.
- Supervise and mentor PTAs, ensuring adherence to therapy protocols and performance standards.
- Maintain compliance with documentation standards, including timely updates and accurate records.
Job Requirements
- Bachelor’s Degree in Physical Therapy; Doctorate in Physical Therapy (DPT) preferred.
- Active Physical Therapist License in New Jersey and AHA Basic Life Support Certification.
- Minimum of 6 months of Physical Therapy experience; 3 years preferred.
- Strong clinical judgment and patient assessment skills for effective therapy planning.
- Proficiency in basic computer systems and electronic medical records (EMR).
- Ability to work independently in a home health setting while maintaining patient-centered care.
- Physical ability to lift, push, and pull up to 50 lbs and remain active for extended periods.
