Overview
- Provide professional rehabilitation services to patients in their homes, promoting functional independence and quality of life.
- Conduct comprehensive evaluations and develop individualized therapy plans tailored to patient needs.
- Implement goal-oriented therapy programs in compliance with physician orders and regulatory standards.
- Collaborate with interdisciplinary care teams for coordinated patient care and referrals.
- Educate patients and caregivers on exercise programs and functional training strategies.
- Maintain accurate and timely documentation in electronic medical records.
- Adjust interventions based on patient progress and clinical outcomes.
- Ensure care delivery adheres to prescribed frequency and duration requirements.
- Participate in discharge planning to support patient transitions.
Key Responsibilities & Duties
- Perform in-home patient evaluations within required timeframes.
- Develop functional treatment plans with measurable short- and long-term goals.
- Deliver skilled occupational therapy services according to physician plans.
- Maintain precise and timely documentation, including evaluations and progress notes.
- Monitor patient progress and adapt interventions as needed.
- Educate patients and caregivers on home exercise programs and strategies.
- Collaborate with physicians and care teams to coordinate services.
- Ensure compliance with organizational policies and regulatory standards.
- Maintain productivity standards in a part-time home care setting.
Job Requirements
- Bachelor’s Degree in Occupational Therapy is required; Master’s Degree preferred.
- Active Occupational Therapist License in New Jersey.
- Minimum of 1 year of occupational therapy experience; 3 years preferred.
- AHA Basic Life Support (BLS) Certification is mandatory.
- Proficiency in electronic medical records and basic computer skills.
- Experience in home care settings is advantageous.
- Physical ability to lift, push, and pull up to 50 lbs.
- Capability to stand and perform clinical duties for extended periods.
- Strong communication skills for effective patient and team interactions.
