Job Description

Overview

  • Provide case management services to ensure patients receive appropriate care tailored to their specific needs.
  • Collaborate with multidisciplinary teams to promote positive health outcomes for patients.
  • Engage patients and families to build trusting relationships and address healthcare needs.
  • Coordinate care plans and link patients with additional services as required.
  • Maintain accurate and organized patient records in compliance with regulations.
  • Participate in quality improvement initiatives and training sessions.
  • Support patients in managing emotional distress and resolving crisis situations.
  • Promote services and recruit patients from underserved communities.

Key Responsibilities & Duties

  • Conduct comprehensive assessments and develop individualized care plans for patients.
  • Collaborate with multidisciplinary teams to monitor and evaluate patient progress.
  • Coordinate with external service providers to ensure continuity of care.
  • Provide clinic-based health education during scheduled primary care visits.
  • Obtain prior authorizations for pharmacy or medical services as required.
  • Respond to inquiries and provide information about available services.
  • Participate in internal and external meetings and training sessions.
  • Ensure compliance with organizational policies and regulatory requirements.

Job Requirements

  • Bachelor’s degree in health, nursing, mental health, or related field required.
  • Minimum of 2 years of experience in case management or related roles.
  • Fluency in Spanish or an Asian language is preferred.
  • Strong written and verbal communication skills in English required.
  • Proficiency in Microsoft Office and healthcare applications is essential.
  • UAS-NY certification required within one month of hire.
  • Ability to work flexible hours, including evenings and weekends.
  • Experience working with diverse populations and underserved communities preferred.
