Overview
- Provide case management services to ensure patients receive appropriate care tailored to their specific needs.
- Collaborate with multidisciplinary teams to promote positive health outcomes for patients.
- Engage patients and families to build trusting relationships and address healthcare needs.
- Coordinate care plans and link patients with additional services as required.
- Maintain accurate and organized patient records in compliance with regulations.
- Participate in quality improvement initiatives and training sessions.
- Support patients in managing emotional distress and resolving crisis situations.
- Promote services and recruit patients from underserved communities.
Key Responsibilities & Duties
- Conduct comprehensive assessments and develop individualized care plans for patients.
- Collaborate with multidisciplinary teams to monitor and evaluate patient progress.
- Coordinate with external service providers to ensure continuity of care.
- Provide clinic-based health education during scheduled primary care visits.
- Obtain prior authorizations for pharmacy or medical services as required.
- Respond to inquiries and provide information about available services.
- Participate in internal and external meetings and training sessions.
- Ensure compliance with organizational policies and regulatory requirements.
Job Requirements
- Bachelor’s degree in health, nursing, mental health, or related field required.
- Minimum of 2 years of experience in case management or related roles.
- Fluency in Spanish or an Asian language is preferred.
- Strong written and verbal communication skills in English required.
- Proficiency in Microsoft Office and healthcare applications is essential.
- UAS-NY certification required within one month of hire.
- Ability to work flexible hours, including evenings and weekends.
- Experience working with diverse populations and underserved communities preferred.
