Job Description

Overview

  • Provide direct nursing care to residents and patients, ensuring safety and satisfaction.
  • Perform charge nurse responsibilities as assigned, supporting the clinical team.
  • Collaborate with Registered Nurses and Clinical Care Managers for optimal care delivery.
  • Work on-site in a structured healthcare environment with rotating weekend shifts.
  • Commit to a minimum of three shifts per week, including weekends.
  • Utilize practical nursing skills to address the needs of a geriatric population.
  • Maintain compliance with healthcare standards and regulations.
  • Participate in continuous education and certification updates as required.

Key Responsibilities & Duties

  • Administer medications and treatments as prescribed by healthcare providers.
  • Monitor patient conditions and report changes to the clinical team.
  • Document patient care activities and maintain accurate medical records.
  • Coordinate with interdisciplinary teams to develop and implement care plans.
  • Provide emotional support and education to patients and their families.
  • Ensure adherence to infection control and safety protocols.
  • Assist in training and mentoring junior staff members.
  • Participate in quality improvement initiatives and audits.

Job Requirements

  • Valid NYS Licensed Practical Nurse (LPN) license.
  • Completion of an approved practical nursing program.
  • BCLS and IV certification required.
  • Experience working with geriatric populations preferred.
  • High School Diploma or GED required.
  • Commitment to working every other weekend.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team-oriented healthcare environment.
