Job Description

Overview

  • Lead clinical operations for a transitional services program supporting youth with complex mental health needs.
  • Collaborate with interdisciplinary teams to ensure effective therapeutic and educational support for clients.
  • Oversee transitions and discharge planning to ensure successful community reintegration.
  • Provide clinical supervision to enhance the competencies of transitional and family support teams.
  • Develop and implement permanency programs to reduce length of stay and minimize re-referrals.
  • Ensure compliance with regulations and guidelines, maintaining high standards of care.
  • Facilitate communication between families, case managers, and community resources.
  • Contribute to a mission-driven organization focused on tailored, comprehensive care.

Key Responsibilities & Duties

  • Supervise and evaluate the performance of transitional coordinators and family support specialists.
  • Coordinate with community-based programs to ensure efficient support for families and youth.
  • Participate in clinical and treatment meetings, providing updates and insights on progress.
  • Develop progressive home time plans and behavior management techniques for reunification success.
  • Maintain collaborative relationships with local resources and referral systems.
  • Ensure proper utilization of transitional funds following established policies.
  • Support compliance with documentation requirements through chart reviews and community visits.
  • Contribute to strategic planning and implementation as a key member of the management team.

Job Requirements

  • Master’s degree in Social Work or related human services field required.
  • Minimum of 2 years of experience providing direct services to youth with emotional disturbances.
  • At least 2 years of supervisory or managerial experience in social services.
  • LCSW certification required for this role.
  • Experience in residential settings and working with youth with mental health challenges preferred.
  • Proficiency in electronic health records and familiarity with ACS and housing systems.
  • Ability to conduct home visits and navigate systems impacting youth and families.
  • Strong interpersonal skills, adaptability, and dedication to supporting youth and families.
