Overview

  • Deliver compassionate hospice nursing care to patients in home settings, ensuring dignity and comfort during end-of-life care.
  • Conduct comprehensive patient assessments, care planning, and implementation aligned with physician orders and best practices.
  • Collaborate with interdisciplinary teams to provide seamless care coordination and communication for optimal patient outcomes.
  • Document clinical decisions and interventions accurately using electronic medical records systems to maintain compliance.
  • Supervise and evaluate home health aide staff, ensuring adherence to care standards and protocols.
  • Participate in interdisciplinary team meetings to discuss patient care plans and updates effectively.
  • Stay informed on clinical standards and hospice care best practices to deliver high-quality patient support.
  • Maintain compliance with organizational policies, physician orders, and regulatory requirements in all care activities.

Key Responsibilities & Duties

  • Provide skilled nursing care tailored to hospice patients' unique needs, ensuring comfort and dignity during end-of-life care.
  • Monitor patient progression and address evolving care requirements with compassion and professionalism.
  • Ensure accurate and timely documentation within electronic medical records systems to support care continuity.
  • Coordinate care services with interdisciplinary teams to ensure seamless implementation of care plans.
  • Supervise home health aide staff, conducting regular evaluations and providing guidance to maintain care quality.
  • Act as a liaison between physicians, interdisciplinary teams, and management to communicate patient updates effectively.
  • Participate in team meetings to contribute to care coordination and decision-making processes.
  • Adhere to organizational policies and regulatory standards in all aspects of hospice care delivery.

Job Requirements

  • Bachelor of Science in Nursing (BSN) or equivalent educational background required.
  • Active NJ Registered Nurse License and AHA Basic Life Support certification mandatory.
  • Minimum of 1 year of RN experience; hospice or home care experience preferred.
  • Proficiency in electronic medical records systems and basic computer skills essential.
  • Strong clinical judgment and ability to work independently in patient care settings.
  • Flexibility to work assigned schedules, including weekends and holidays, as needed.
  • Commitment to maintaining compassionate, safe, and effective patient care standards.
  • Knowledge of hospice care principles and regulatory compliance requirements preferred.
