Overview
- Deliver compassionate nursing care in a dynamic adult day care environment, focusing on registrants' health and wellness.
- Collaborate with a multidisciplinary team to develop and implement personalized care plans for registrants.
- Conduct thorough nursing assessments and monitor registrants' health status to ensure optimal care delivery.
- Educate registrants on health topics, empowering informed decisions and promoting wellness.
- Manage medication schedules and inventory, ensuring compliance with healthcare standards.
- Act as a liaison between registrants, their physicians, and the program's Medical Director to coordinate care.
- Participate in quality assurance initiatives to improve care standards and outcomes.
- Provide assistance with personal care and mobility to support registrants' daily activities.
Key Responsibilities & Duties
- Perform comprehensive nursing assessments and create individualized treatment plans tailored to registrants' needs.
- Monitor medication schedules and maintain accurate inventory records to ensure compliance.
- Respond promptly to medical emergencies and educate staff on safety protocols.
- Provide health counseling and lead group education sessions to promote registrants' understanding of health topics.
- Supervise nursing students and volunteers, fostering effective training and mentorship.
- Collaborate with interdisciplinary teams to optimize registrants' care plans and outcomes.
- Participate in staff meetings and contribute to program development and improvement initiatives.
- Assist registrants with mobility, meals, and transportation arrangements to support their daily needs.
Job Requirements
- Associate of Applied Science (AAS) degree from an accredited nursing institution.
- Current NYS Registered Nurse license; UAS-NY certification preferred.
- 3-5 years of experience in Med/Surg, Long Term Care, or Community Health settings.
- Proficiency in MS Word and strong documentation skills are essential.
- Excellent communication skills and ability to engage effectively with elderly populations.
- Physical capability to assist registrants with mobility and equipment handling.
- Commitment to confidentiality and adherence to health information security policies.
- Experience in interdisciplinary collaboration and program development is preferred.
