Job Description

Overview

  • Provide specialized nursing care in a detox setting, addressing medical and psychiatric needs to ensure optimal client health and recovery.
  • Collaborate with multidisciplinary teams to create and implement personalized care plans tailored to individual client requirements.
  • Deliver health education and advocacy, empowering clients to manage their health effectively and independently.
  • Maintain accurate and detailed documentation of client interactions, medical records, and care plans for seamless communication.
  • Provide emergency medical assistance, including first aid and CPR, ensuring immediate response to critical situations.
  • Participate actively in team meetings to share insights, coordinate care strategies, and enhance service delivery.
  • Ensure compliance with healthcare protocols and standards to uphold high-quality care delivery and client safety.

Key Responsibilities & Duties

  • Supervise and guide staff to ensure adherence to medical and psychosocial care protocols and procedures.
  • Coordinate with healthcare providers and pharmacies to manage client medications and treatments effectively.
  • Conduct routine health assessments, monitor client vitals, and track progress to inform care decisions.
  • Document findings and updates in team meetings, ensuring comprehensive communication and collaboration.
  • Educate clients on health management practices and advocate for their preferences and well-being.
  • Perform medication counts and ensure adequate supplies to meet client needs and maintain continuity of care.
  • Assist clients with practical needs, such as understanding bills or accessing essential services.
  • Provide immediate medical assistance during emergencies, including CPR and first aid, ensuring client safety.

Job Requirements

  • Active Licensed Practical Nurse (LPN) license required, demonstrating professional competency and readiness.
  • Associate of Science (AS) degree in nursing or related field, ensuring foundational knowledge.
  • Minimum of 1 year of nursing experience required; 3 years preferred for advanced proficiency.
  • Proficiency in medical documentation and record-keeping practices to ensure accurate client data management.
  • Strong problem-solving skills to address complex client needs effectively and efficiently.
  • Excellent communication skills for client advocacy and effective collaboration with multidisciplinary teams.
