Overview

  • Provide comprehensive physical and psychosocial assessments for hospice patients, ensuring personalized care plans addressing end-of-life needs.
  • Collaborate within an interdisciplinary team to deliver high-quality, patient-centered hospice care.
  • Supervise and train home health aides, homemakers, and personal care workers in patients' homes.
  • Request and manage physician orders for adjustments to patient care plans as necessary.
  • Ensure accurate and timely documentation of clinical data and patient progress.
  • Support patients and families through effective communication and education on hospice care processes.
  • Maintain compliance with healthcare regulations and standards in hospice care delivery.
  • Contribute to continuous improvement initiatives within the hospice care program.

Key Responsibilities & Duties

  • Perform detailed physical and psychosocial assessments for hospice patients, tailoring care plans to individual needs.
  • Develop and implement nursing care plans, ensuring alignment with patient goals and interdisciplinary team recommendations.
  • Supervise and evaluate the performance of home health aides and other support staff.
  • Request physician orders for necessary changes in patient care plans, ensuring optimal treatment outcomes.
  • Provide education and guidance to patients and families regarding hospice care processes and expectations.
  • Ensure accurate documentation of patient care activities and clinical data in compliance with regulatory standards.
  • Participate in interdisciplinary team meetings to discuss patient progress and care strategies.
  • Stay updated on best practices and advancements in hospice care to enhance service delivery.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or equivalent educational background.
  • Minimum of 2 years of clinical experience, preferably in hospice, home care, or acute care settings.
  • Proficiency in clinical data documentation and electronic health record systems.
  • Strong communication, teaching, and interpersonal skills for effective patient and team interactions.
  • Ability to work collaboratively within an interdisciplinary team environment.
  • Commitment to delivering compassionate, patient-centered hospice care.
  • Knowledge of hospice care regulations and standards is advantageous.
