Key Responsibilities & Duties
- Guide patients and families through program orientation and treatment planning processes.
- Coordinate education sessions for patients and caregivers to encourage active participation in care.
- Develop and implement individualized treatment plans tailored to patient recovery goals.
- Conduct psychosocial assessments and document findings for interdisciplinary team collaboration.
- Collaborate with program directors and case management leaders to ensure seamless care delivery.

Job Requirements
- Associate Degree in Nursing or Registered Nurse Diploma required.
- Minimum of 2 years of recent acute care case management experience preferred.
- Active Registered Nurse (RN) license or Advanced Practice Registered Nurse (APRN) certification.
- Strong knowledge of rehabilitation philosophy and insurance benefits.
- Proficiency in developing and implementing patient-centered care plans.
- Excellent communication and collaboration skills with interdisciplinary teams.
