Overview

  • Lead case management initiatives in a dynamic healthcare environment, ensuring patient-centered care and seamless transitions.
  • Coordinate with interdisciplinary teams to develop and implement individualized care plans for patients.
  • Support patients and families through their care journey, providing education and resources.
  • Promote excellence in patient care by adhering to healthcare standards and organizational values.
  • Collaborate with department leaders to enhance care quality and operational efficiency.
  • Engage in discharge planning evaluations and psychosocial assessments to ensure comprehensive care.
  • Contribute to the development of rehabilitation programs and patient education initiatives.
  • Utilize your acute care experience to drive impactful case management strategies.

Key Responsibilities & Duties

  • Guide patients and families through program orientation and treatment planning processes.
  • Coordinate education sessions for patients and caregivers to encourage active participation in care.
  • Develop and implement individualized treatment plans tailored to patient recovery goals.
  • Conduct psychosocial assessments and document findings for interdisciplinary team collaboration.
  • Collaborate with program directors and case management leaders to ensure seamless care delivery.
  • Promote organizational values of compassion, respect, and excellence in patient interactions.
  • Monitor and evaluate patient progress, adjusting care plans as needed for optimal outcomes.
  • Ensure compliance with healthcare regulations and standards in all case management activities.

Job Requirements

  • Associate Degree in Nursing or Registered Nurse Diploma required.
  • Minimum of 2 years of recent acute care case management experience preferred.
  • Active Registered Nurse (RN) license or Advanced Practice Registered Nurse (APRN) certification.
  • Strong knowledge of rehabilitation philosophy and insurance benefits.
  • Proficiency in developing and implementing patient-centered care plans.
  • Excellent communication and collaboration skills with interdisciplinary teams.
  • Ability to conduct psychosocial assessments and discharge planning evaluations effectively.
