Job Description

Overview

  • Provide nursing care and coordinate medical services for individuals in assigned sites, ensuring comprehensive health management.
  • Perform assessments, plan interventions, and evaluate outcomes of care to meet individual health needs effectively.
  • Administer medications and conduct specialized medical treatments as mandated by care plans.
  • Offer 24/7 medical consultation and telephone triage for agency staff and individuals.
  • Supervise and provide training for AMAP-certified staff, ensuring adherence to medical protocols.
  • Coordinate medical services with residences, schools, and day services to ensure seamless care delivery.
  • Maintain updated knowledge through in-service training on medical care and developmental disabilities.
  • Implement care plans addressing medical profiles, including ADL skills and behavior modification techniques.

Key Responsibilities & Duties

  • Conduct thorough assessments of medical needs and develop individualized care plans for residents.
  • Coordinate with medical providers and dieticians to ensure comprehensive care management.
  • Provide direct care, including assistance with daily living activities and hygiene maintenance.
  • Administer medications and ensure proper documentation of medical interventions.
  • Monitor and supervise AMAP-certified staff, conducting certification and recertification classes.
  • Coordinate nutrition services and assist with menu preparation to meet dietary needs.
  • Attend training sessions to stay updated on best practices in medical care for developmental disabilities.
  • Maintain accurate records, including monthly progress notes and medical documentation.

Job Requirements

  • Bachelor of Science in Nursing (BSN) preferred; Associate degree accepted.
  • Current First Aid and CPR certifications are mandatory.
  • Minimum of 2 years of experience working with adults with developmental disabilities.
  • Completion of mandated courses in Infection Control and Abuse/Incident Reporting.
  • Strong commitment to safety and adherence to medical protocols.
  • Ability to provide 24/7 medical consultation and triage services.
  • Proficiency in supervising and training medical staff effectively.
  • Excellent organizational and communication skills for coordinating care services.
