Job Description

Overview

  • Support coordination of home health aide services for hospice and home care patients in a hybrid work environment.
  • Ensure timely processing of service requests and maintain effective communication with clinical teams and vendors.
  • Accurately document service authorizations within internal systems and HHA Exchange platforms.
  • Collaborate with clinical teams to review and verify documentation for service coordination.
  • Handle inbound calls and provide prompt assistance to inquiries regarding services.
  • Work in a dynamic environment with opportunities to enhance operational efficiency.
  • Engage in problem-solving and critical thinking to address service-related challenges.

Key Responsibilities & Duties

  • Process incoming service requests from clinical teams with accuracy and efficiency.
  • Review and validate requests before assigning cases to vendors.
  • Assign approved cases to vendors using the HHA Exchange platform.
  • Ensure accurate entry of service authorizations in internal systems.
  • Communicate updates and confirmations with vendors and clinical teams promptly.
  • Monitor clinical documentation in EMR to support service coordination.
  • Verify patient hospital stays or releases and notify vendors accordingly.
  • Run discharge reports and notify vendors about patient status changes.
  • Answer calls on ACD line and provide assistance to inquiries.

Job Requirements

  • High School Diploma or GED required; Associate degree preferred.
  • Minimum 1 year of experience; 2 years preferred in a related role.
  • Proficiency in Microsoft Office applications, including Excel, Word, and Outlook.
  • Strong attention to detail and accuracy in data entry tasks.
  • Excellent communication skills for effective collaboration and service coordination.
  • Ability to prioritize tasks and work efficiently in a fast-paced environment.
  • Problem-solving and critical thinking skills to address operational challenges.
  • Ability to work independently and collaboratively within a team setting.
