Job Description

Overview

  • Support placement and coordination of home health aide services for hospice and home care patients.
  • Ensure timely processing of service requests and effective communication with clinical teams and vendors.
  • Utilize internal systems and HHA Exchange platforms for accurate documentation and service authorization.
  • Collaborate within a hybrid work schedule for optimal efficiency and productivity.
  • Engage in a contract-based role with opportunities to develop administrative skills.
  • Contribute to a dynamic healthcare environment focusing on patient care coordination.

Key Responsibilities & Duties

  • Process incoming requests for home health aide services from clinical teams efficiently.
  • Review and verify completeness and accuracy of service requests before vendor assignment.
  • Assign approved cases to contracted vendors using the HHA Exchange platform.
  • Enter accurate service authorizations and maintain updated records in internal systems.
  • Communicate service updates and confirmations with vendors and clinical teams promptly.
  • Review clinical documentation and verify hospital stays or releases for service coordination.
  • Run daily discharge reports and notify vendors of patient deaths or discharges.
  • Answer inbound calls on ACD line and provide necessary assistance.

Job Requirements

  • High School Diploma or GED required; Associate degree preferred.
  • Minimum of 1 year experience in administrative or healthcare coordination roles.
  • Proficiency in Microsoft Office Suite, including Excel, Word, and Outlook.
  • Strong communication skills and ability to work in a fast-paced environment.
  • Attention to detail and accuracy in data entry and documentation.
  • Ability to prioritize tasks and solve problems effectively.
  • Experience with HHA Exchange platforms is highly desirable.
  • Capability to work independently and collaboratively within a team.
