Job Description

Overview

  • Support coordination of home health aide services for hospice and home care patients in a hybrid work environment.
  • Ensure timely processing of service requests and maintain effective communication with clinical teams and vendors.
  • Utilize HHA Exchange platform for accurate documentation and service authorization management.
  • Collaborate with clinical teams to review and verify service requests for completeness and accuracy.
  • Provide updates on service status and confirmations to vendors and clinical teams promptly.
  • Perform data entry and documentation tasks with attention to detail and accuracy.
  • Handle inbound calls efficiently and address inquiries professionally.
  • Maintain compliance with organizational policies and procedures.

Key Responsibilities & Duties

  • Process incoming requests for home health aide services and ensure accuracy before offering cases to vendors.
  • Assign approved cases to contracted vendors using the HHA Exchange platform.
  • Enter accurate service authorizations in the HHA Exchange platform.
  • Review clinical documentation in EMR systems to support service coordination.
  • Verify hospital stays or releases and run daily discharge reports.
  • Notify vendors promptly regarding patient discharges or deaths.
  • Communicate service updates and confirmations with vendors and clinical teams.
  • Answer inbound calls on ACD lines and provide professional assistance.

Job Requirements

  • High School Diploma or GED required; additional certifications in healthcare coordination preferred.
  • Minimum of 1 year of experience in healthcare coordination; 2 years preferred.
  • Proficiency in Microsoft Office Suite, including Excel, Outlook, and Word.
  • Strong communication skills and ability to work in a fast-paced environment.
  • Attention to detail and accuracy in data entry and documentation.
  • Ability to prioritize tasks and manage multiple responsibilities effectively.
  • Problem-solving and critical thinking skills to address service coordination challenges.
  • Experience with EMR systems and HHA Exchange platform is advantageous.
