Key Responsibilities & Duties
- Conduct thorough assessments, diagnoses, and treatments for patients with complex health needs.
- Lead and coordinate care teams to ensure seamless patient care delivery.
- Engage in proactive communication with patients, addressing concerns and providing education.
- Collaborate with specialists and external providers to optimize patient outcomes.
- Ensure compliance with clinical guidelines, regulatory standards, and organizational policies.
- Document patient interactions and care plans accurately using clinic systems.

- Support recruitment and mentoring of new providers within the organization.
Job Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Active state medical license and DEA certification for controlled substances.
- Board certification in Internal Medicine, Family Medicine, or Geriatrics.
- Minimum of 1 year of experience; 5 years preferred in primary care settings.
- Proficiency in electronic medical records and clinical documentation systems.
- Strong commitment to patient-centered care and health outcome accountability.
- Ability to work effectively in a multidisciplinary team environment.
- Basic Life Support (BLS) certification within 90 days of employment.
