Job Description

Overview

  • Provide comprehensive primary care services to a diverse patient population in an on-site setting.
  • Collaborate with a multidisciplinary team to deliver value-based care and improve patient outcomes.
  • Focus on holistic care, addressing both acute and chronic health conditions effectively.
  • Engage patients in setting and achieving personalized health goals through coaching and support.
  • Manage a patient panel of approximately 400 individuals, ensuring quality care delivery.
  • Utilize technology efficiently for documentation and care coordination within the clinic.

Key Responsibilities & Duties

  • Conduct thorough assessments, diagnoses, and treatments for patients with complex health needs.
  • Lead and coordinate care teams to ensure seamless patient care delivery.
  • Engage in proactive communication with patients, addressing concerns and providing education.
  • Collaborate with specialists and external providers to optimize patient outcomes.
  • Ensure compliance with clinical guidelines, regulatory standards, and organizational policies.
  • Document patient interactions and care plans accurately using clinic systems.
  • Participate in quality improvement initiatives and risk management programs.
  • Support recruitment and mentoring of new providers within the organization.

Job Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
  • Active state medical license and DEA certification for controlled substances.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics.
  • Minimum of 1 year of experience; 5 years preferred in primary care settings.
  • Proficiency in electronic medical records and clinical documentation systems.
  • Strong commitment to patient-centered care and health outcome accountability.
  • Ability to work effectively in a multidisciplinary team environment.
  • Basic Life Support (BLS) certification within 90 days of employment.
