Job Description

Overview

  • Serve as a Primary Care Physician providing comprehensive healthcare to patients.
  • Focus on delivering value-based care to underserved communities.
  • Work closely with a multidisciplinary team to ensure optimal patient outcomes.
  • Engage in holistic care practices, including health coaching and lifestyle interventions.
  • Maintain a manageable patient panel size to enhance care quality.
  • Utilize technology for efficient documentation and patient management.
  • Participate in center operations and contribute to a positive culture.
  • Adhere to departmental goals, policies, and regulatory compliance standards.

Key Responsibilities & Duties

  • Provide care for patients with acute and chronic illnesses, focusing on older adults.
  • Take accountability for patient outcomes and collaborate with specialists as needed.
  • Engage with hospitalists for patients requiring hospital care.
  • Lead care teams, including medical assistants and care coordinators.
  • Participate in transitional care for homebound or institutionalized patients.
  • Contribute to recruiting efforts for patients and additional providers.
  • Supervise and collaborate with Advanced Practice Practitioners.
  • Engage in quality improvement initiatives and promote evidence-based practices.

Job Requirements

  • Doctor of Medicine (MD) degree with board certification in Internal Medicine, Family Medicine, or Geriatrics.
  • Active State Medical License or eligibility to obtain one within the required timeframe.
  • Current DEA number for controlled substances and Basic Life Support certification.
  • Proficiency in Microsoft Office Suite and other healthcare software.
  • Ability to travel locally for patient care and nationally for training.
  • Fluency in English, with additional language skills preferred for specific centers.
  • Strong commitment to patient-centered care and service orientation.
  • Experience in value-based care and managing patient health outcomes.
