Job Description

Overview

  • Provide strategic leadership for care management functions, including case management, utilization management, and discharge planning.
  • Ensure patient outcomes, satisfaction, and care continuum utilization meet or exceed benchmarks.
  • Develop and execute strategic plans aligned with organizational goals and the Triple Aim.
  • Collaborate with physicians and medical staff to promote regulatory compliance and utilization practices.
  • Lead initiatives to improve quality, outcomes, and patient experience across care delivery.
  • Supervise and develop leaders and caregivers, fostering a culture of collaboration and excellence.
  • Manage departmental budgets, staffing plans, and operational metrics for financial stewardship.
  • Ensure compliance with licensing, accreditation, and regulatory standards.

Key Responsibilities & Duties

  • Oversee care transitions, utilization management, and discharge planning processes.
  • Monitor departmental performance metrics and address operational challenges proactively.
  • Recruit, mentor, and retain high-performing teams within areas of responsibility.
  • Collaborate with community care partners to enhance patient care coordination.
  • Develop annual strategic work plans to achieve clinical, administrative, and financial objectives.
  • Lead reporting efforts regarding care management outcomes and performance.
  • Ensure responsible utilization of resources while maintaining quality outcomes.
  • Participate in hospital and medical staff committees as required.

Job Requirements

  • Bachelor's degree in nursing and a master's degree in a health-related field.
  • Minimum of 2 years of care management experience in an acute-care setting.
  • At least 2 years of healthcare leadership experience related to case management.
  • Active California Registered Nurse (RN) License.
  • California Fire and Life Safety Card within 30 days of hire.
  • Proficiency in Microsoft Office applications, including Word, Excel, PowerPoint, and Outlook.
  • Strong knowledge of acute-care case management principles and practices.
  • Ability to manage multiple priorities in a fast-paced healthcare environment.
