Job Description

Overview

  • Provide compassionate, patient-centered care to individuals in hospice, ensuring dignity and comfort during end-of-life stages.
  • Conduct comprehensive assessments and develop personalized care plans addressing physical, emotional, and psychosocial needs.
  • Collaborate with interdisciplinary teams to deliver holistic and coordinated care for patients and their families.
  • Educate patients and families about hospice services, care processes, and available support resources.
  • Supervise and mentor home health aides, ensuring adherence to quality standards and best practices.
  • Maintain meticulous documentation of patient care activities, clinical data, and progress updates.
  • Ensure compliance with hospice care policies, standards, and regulatory requirements at all times.
  • Provide emotional support to patients and families, fostering a compassionate and supportive environment.

Key Responsibilities & Duties

  • Conduct initial and ongoing assessments to evaluate hospice patient care needs and conditions.
  • Develop and implement individualized nursing care plans tailored to patient health status and preferences.
  • Coordinate with physicians and interdisciplinary teams to ensure seamless communication and care delivery.
  • Monitor patient progress and adapt care plans to address evolving needs and conditions effectively.
  • Provide education and emotional support to patients and families regarding hospice care and coping strategies.
  • Supervise and train home health aides and personal care workers to uphold quality care standards.
  • Ensure accurate and timely documentation of patient care activities and clinical observations.
  • Maintain compliance with hospice care standards, policies, and regulatory requirements consistently.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or related field required.
  • Minimum of 1 year of clinical nursing experience, preferably in hospice or acute care settings.
  • Preferred 6 years of experience in hospice or related nursing roles.
  • Strong ability to collaborate effectively within interdisciplinary team environments.
  • Proficiency in clinical data input and maintaining accurate documentation.
  • Excellent communication, teaching, and listening skills to support patients and families.
  • Knowledge of hospice care standards, practices, and regulatory requirements preferred.
