Admissions Nurse – Hospice Key Responsibilities & Duties

• Manage high-tech and complex referrals within the Access Center to ensure timely admissions to the hospice program.
• Collaborate with Access Coordinators to monitor referral lists and facilitate efficient admissions processes.
• Communicate effectively with interdisciplinary teams to ensure safe and comprehensive patient care.
• Verify all orders for evaluation prior to admission to ensure compliance and readiness.
• Provide excellent customer service and maintain clear written and oral communication with stakeholders.
• Utilize PC skills to manage electronic records and documentation efficiently.
• Adapt to various patient needs and conditions, leveraging acute care experience for effective hospice admissions.
• Participate in occasional weekend coverage to ensure continuous patient care and support.

Admissions Nurse – Hospice Job Requirements

• Graduate from an accredited school of Nursing with a valid NY State RN License.
• Bachelor of Science in Nursing (BSN) preferred but not mandatory.
• Minimum of 2 years of acute care experience; hospice experience is not mandatory.
• Experience in ER, Critical Care, Oncology, or Long-Term Care is highly valued.
• Proficiency in PC skills and electronic documentation systems is required.
• Strong written and oral communication skills to provide excellent customer service.
• Access to a reliable vehicle for travel within Brooklyn and the Bronx.
• Ability to work onsite from 11AM to 7PM with occasional weekend shifts.
