Primary Care Provider Overview

  • The Primary Care Provider will deliver patient-centered care to diverse populations, emphasizing respect and compassion.
  • Provide comprehensive primary care services, including screenings, diagnoses, and treatments across the lifespan.
  • Collaborate within a multidisciplinary team to ensure coordinated and effective patient care.
  • Utilize evidence-based guidelines to deliver unbiased and high-quality healthcare services.
  • Maintain accurate and timely documentation in the electronic health record system.
  • Participate in quality improvement initiatives and professional development opportunities.
  • Flexible scheduling options, including daytime and evening shifts, with optional Saturdays.

Primary Care Provider Key Responsibilities & Duties

  • Conduct patient assessments, screenings, and examinations to diagnose and treat various health conditions.
  • Provide compassionate and respectful care consistent with evidence-based practices.
  • Collaborate with healthcare professionals to ensure coordinated and effective patient care.
  • Document patient care promptly and accurately in the electronic health record system.
  • Engage in quality improvement initiatives and participate in case conferences.
  • Deliver care during scheduled appointments and ensure timely follow-up on patient needs.
  • Make appropriate referrals to internal and external healthcare providers.
  • Participate in continuing medical education to enhance professional skills.
  • Support the organization's mission to provide accessible healthcare to LGBTQ+ communities.

Primary Care Provider Job Requirements

  • Doctor of Medicine (MD) degree with New York State Certification/Licensure.
  • Current NYS Registration and DEA Controlled Substances Registration Certificate.
  • Board certification or eligibility in Family Medicine, Internal Medicine, or Infectious Disease.
  • Minimum of 2 years of primary care experience; 3 years preferred.
  • Experience in community-based primary care and HIV/transgender care is a plus.
  • Strong collaboration skills within a team-based care model.
  • Commitment to accessible, patient-centered healthcare delivery.
  • Bilingual proficiency in Spanish/English is advantageous.
  • Current CPR Certification is required.
