Community Liaison Key Responsibilities & Duties
- Establish and maintain relationships with healthcare professionals to promote hospice services.
- Conduct outreach activities to increase awareness and preference for hospice care.
- Drive census growth by engaging with referral sources and community partners.
- Support patients and families by explaining hospice services and facilitating consent.
- Collaborate with clinical teams to ensure seamless patient care transitions.
- Participate in community events to represent the organization and its mission.

Community Liaison Job Requirements
- Bachelor’s degree in a related field preferred; equivalent experience considered.
- Minimum of 2 years in healthcare sales/marketing; hospice experience preferred.
- Proven ability to build and maintain relationships with healthcare professionals.
- Strong understanding of hospice services and patient care principles.
- Reliable transportation, valid driver’s license, and auto insurance required.
- Excellent communication and interpersonal skills for effective outreach.
- Ability to work independently and manage a hybrid work schedule.
