Community Liaison Key Responsibilities & Duties

• Establish and maintain relationships with healthcare professionals to promote hospice services.
• Conduct outreach activities to increase awareness and preference for hospice care.
• Drive census growth by engaging with referral sources and community partners.
• Support patients and families by explaining hospice services and facilitating consent.
• Collaborate with clinical teams to ensure seamless patient care transitions.
• Participate in community events to represent the organization and its mission.
• Provide feedback to improve service delivery and outreach strategies.
• Maintain accurate records of outreach activities and referral outcomes.

Community Liaison Job Requirements

• Bachelor’s degree in a related field preferred; equivalent experience considered.
• Minimum of 2 years in healthcare sales/marketing; hospice experience preferred.
• Proven ability to build and maintain relationships with healthcare professionals.
• Strong understanding of hospice services and patient care principles.
• Reliable transportation, valid driver’s license, and auto insurance required.
• Excellent communication and interpersonal skills for effective outreach.
• Ability to work independently and manage a hybrid work schedule.
• Commitment to the mission of providing compassionate end-of-life care.
