Pediatric Palliative Care Nurse Practitioner Overview

  • The Pediatric Palliative Care Nurse Practitioner provides specialized care to pediatric patients requiring hospice and palliative services.
  • Conduct in-person and telehealth visits, ensuring comprehensive and compassionate care delivery.
  • Collaborate with interdisciplinary teams to develop and implement effective care plans.
  • Provide training and support to hospice teams on pediatric care practices.
  • Utilize evidence-based approaches for symptom management and communication strategies.
  • Contribute to quality improvement initiatives and curriculum development for pediatric hospice care.
  • Ensure compliance with best practices in history-taking, examination, treatment planning, and documentation.
  • Maintain accountability and demonstrate effective critical thinking in patient care.

Pediatric Palliative Care Nurse Practitioner Key Responsibilities & Duties

  • Perform face-to-face evaluations for hospice eligibility and exigent medical care management.
  • Coordinate with team physicians and managers to ensure optimal patient outcomes.
  • Provide direct care and support to clinicians using evidence-based practices.
  • Assist in staffing pediatric palliative care consultation services as needed.
  • Develop and implement teaching curricula for pediatric hospice care.

Pediatric Palliative Care Nurse Practitioner Job Requirements

  • Master’s degree in nursing and completion of an approved Nurse Practitioner Program.
  • Minimum of 2 years of clinical experience, with 4 years preferred.
  • New York State RN licensure and Certified Nurse Practitioner certification required.
  • CPR-BLS certification and valid NYS Driver's license preferred.
  • Hospice & Palliative Care certification encouraged within 2 years of hire.
  • Proficiency in Microsoft Word and Excel for documentation purposes.
  • Strong communication, teaching, and organizational skills essential.
  • Ability to work effectively within an integrated healthcare team.
